Apple Cider Vinegar for Stabilizing Blood Glucose?

Apple cider vinegar (ACV) shows up in a lot of health videos. People say it can steady blood sugar. Some of that is true. Some of it is oversold. Here is what the research actually shows, in plain language.

ACV is apple juice that has been fermented twice. First it turns into alcohol. Then it turns into vinegar. The main working part is acetic acid, the same sour acid found in other vinegars. A typical bottle is about 5% to 6% acetic acid. The “mother” you see floating in raw ACV is leftover yeast and bacteria. It is not magic. The acid is what researchers study.

How it may affect blood sugar

After you eat carbs, enzymes in your gut break starch into sugar. That sugar then moves into your blood. Acetic acid can slow those enzymes. It can also slow how fast your stomach empties. Food sits a little longer, so sugar enters the blood more slowly. Some research also suggests vinegar may help muscles take in more glucose and may change how the liver makes sugar overnight.

Think of it like a speed bump, not a stop sign. Your blood sugar can still rise. It may just rise less sharply.

What the studies found

The strongest evidence is for blood sugar after a carb-heavy meal. In several small trials, people who drank 1 to 2 tablespoons of vinegar with or right before a starchy meal had a smaller spike than people who drank water. Drops of about 20% to 30% in the early post-meal rise have been reported. The effect is clearer with bread, rice, and other starches than with every kind of food.

Longer-term numbers are more mixed, but they lean in the same direction.

A 2021 review of randomized trials found that ACV lowered fasting blood sugar by about 8 mg/dL and A1C by about 0.5 points. A1C is a lab test that shows average blood sugar over about three months.

A 2025 review looked only at people with type 2 diabetes. It pooled seven trials with 463 people. Fasting blood sugar fell by about 22 mg/dL. A1C fell by about 1.5 points. Effects on fasting sugar looked stronger at doses above 10 mL a day. The authors rated the fasting-sugar evidence as moderate. They were less sure about A1C because the studies were small and many came from one region. Insulin resistance, measured by HOMA-IR, did not clearly change.

Other reviews of vinegar in general (not only ACV) have found similar patterns: fasting glucose down by roughly 9 mg/dL and A1C down by about 0.4 to 0.7 points. Results vary a lot from study to study.

One well-known small trial from Arizona State University tested bedtime use. Adults with type 2 diabetes drank 2 tablespoons of vinegar with a small snack at night. The next morning, fasting sugar was a bit lower. The drop was about 4% overall and about 6% in people whose morning numbers were already high. That is a few points, not a cure.

So the research supports a modest effect. It does not support the idea that ACV can replace medicine, diet, or exercise.

Pros

ACV is cheap and easy to find. You do not need a prescription.

Used with meals, it may blunt the rise in blood sugar after carbs. For some people that means fewer sharp peaks.

Used daily for weeks, it may slightly lower fasting sugar and A1C. The change is usually small. For a person whose A1C is 8.0, a drop of half a point is helpful. It is not the same as going from 8.0 to 6.5.

Some of the same studies also saw small drops in total cholesterol. That is a bonus, not the main reason to use it.

You can use it as food, not just as a “shot.” Salad dressing with vinegar and oil is a simple way to get the acid with a meal.

Cons and risks

ACV is strong acid. Drunk straight, it can burn the throat and wear down tooth enamel. It can also worsen heartburn and upset the stomach. Always dilute it. Use a straw if you drink it, then rinse your mouth with water. Do not brush right away. Acid plus brushing can scrape enamel.

It can slow stomach emptying. That may help blood sugar after a meal. It can also cause bloating, nausea, and uneven blood sugar in people who already have slow stomach emptying, a problem called gastroparesis. People with that condition, especially those with type 1 diabetes, should be careful.

Large amounts used for a long time have been linked to low potassium. Low potassium can cause muscle weakness and heart rhythm problems. The risk rises if you also take water pills (diuretics), digoxin, or insulin, all of which can lower potassium.

Because ACV can lower blood sugar a little, it can add to the effect of diabetes drugs. That includes insulin, sulfonylureas, and some other medicines. Blood sugar could fall too low. If you use these drugs, talk with your doctor first and check your numbers more often.

The taste is harsh. Many people cannot stick with it.

The studies themselves have limits. Most are small. Many last only 8 to 12 weeks. Quality is uneven. A few viral claims about huge weight loss from ACV have even been pulled after the science did not hold up. Blood-sugar data is better than the weight-loss hype, but it is still not ironclad.

ACV is not approved as a diabetes treatment. Health agencies do not list it as standard care.

How to use it if you try it

A common research dose is 1 to 2 tablespoons (15 to 30 mL) a day. Mix it in a large glass of water. Take it with or just before a meal that has carbs, or at bedtime with a small snack if you are targeting morning numbers.

Start with 1 teaspoon in water to see how your stomach handles it. Do not drink it undiluted. Do not take huge daily doses. More is not better, and high doses raise the risk of enamel damage and low potassium.

Pills and gummies exist. Liquid vinegar is what most studies used. Pills may not match the dose or the acid exposure of liquid vinegar.

If you have kidney disease, ulcers, low potassium, or gastroparesis, skip it unless your doctor says otherwise.

The bottom line

Apple cider vinegar can modestly help stabilize blood sugar. The best-supported use is a diluted dose with a carb meal to flatten the spike. Daily use for weeks may slightly lower fasting sugar and A1C in people with type 2 diabetes. The effect is real in many trials, but it is small.

It is a side tool, not the main tool. Medicine, movement, sleep, and what you eat still do the heavy lifting. If you try ACV, dilute it, protect your teeth, watch for stomach trouble, and tell your doctor if you take diabetes drugs. Check your own numbers. If they do not change, do not force a habit that burns your throat for no gain.

A sour drink will not undo a high-sugar diet. Used carefully, it may take a little pressure off the next spike. That is useful. It is not a miracle.
